Histologic findings are those of nonspecific interstitial pneumonitis. Diagnosis of hypersensitivity pneumonitis was based on radiologic and clinical findings. Patient had positive results for Aspergillus precipitins, but specific etiologic agent for hypersensitivity
Hypersensitivity pneumonitis (HP) is an allergic lung disease caused by the inhalation of antigens contained in a variety of organic dusts. Farmer's lung is the best-known HP syndrome and results from the inhalation of fungal organisms that grow in moist hay or exposure to birds as pets (1).
Two multinucleated giant cells can be seen within the interstitium at left, and a plug of organizing pneumonia at bottom left. 2020-08-20 Chronic hypersensitivity pneumonitis with a fibrotic, nonspecific interstitial pneumonia (NSIP) pattern in a patient with bird exposure. The biopsy shows a typical, fibrotic NSIP pattern. A Schaumann body, which is a marker of a previously present granuloma, is illustrated in the inset (hematoxylin-eosin, original magnifications ×25 and ×200 [inset]) . 2018-05-30 INTRODUCTION — Hypersensitivity pneumonitis (HP), also called extrinsic allergic alveolitis, is a complex syndrome of varying intensity, clinical presentation, and natural history, rather than a single, uniform disease [].Numerous inciting agents have been described including, but not limited to, agricultural dusts, bioaerosols, microorganisms (fungal, bacterial, or protozoal), and certain complicated; it is sometimes difficult to differentiate chronic hypersensitivity pneumonitis from idiopathic pulmonary fibrosis/usual interstitial pneumonia, nonspecific interstitial pneumonia, and connective-tissue-related lung disease.
Chest radiographs obtained in many patients with hypersensitivity pneumonitis are normal (47). Abnormal radiographic findings observed in some patients include numerous poorly defined small (less than 5-mm) opacities throughout both lungs, sometimes with sparing of the apices and bases. Hypersensitivity pneumonitis is an immunologically mediated lung disease that occurs in susceptible sensitized individuals following inhalational exposure to antigenic organic dust. As such, the antigenic material must be small enough to deposit in the bronchioles and penetrate the alveoli, where it induces an inflammatory response. Areas of organizing pneumonia also may be seen. The high-resolution CT and pathologic features of chronic HP frequently overlap with those of nonspecific interstitial pneumonia and usual interstitial pneumonia. Awareness of the various manifestations of HP is important for early diagnosis and management.

In some cases, antigenic material is visible, or organisms may grow on culture. As in the acute and subacute forms, lymphocytic peribronchiolar inflammation is also seen in chronic hypersensitivity pneumonitis, but with plasma cell predominance.

~ 10 years among those with bird fancier’s lung) 3. Although the symptomatic disease has been classically divided into acute, subacute, and chronic types, given contradictory definitions, it has been more recently divided in acute/inflammatory type (non-fibroti… Microscopic appearance. Acute hypersensitivity pneumonitis is histologically characterized by the presence of neutrophilic infiltration of the respiratory bronchioles and alveoli. A pattern of diffuse alveolar damage and temporally uniform, non-specific, chronic interstitial pneumonitis may also be seen. Chronic hypersensitivity pneumonitis (CHP) refers to hypersensitivity pneumonitis where there is radiological evidence of fibrosis and represents the end-stage of repeated or persistent pneumonitis 7.
